The parish was established in 1887.  The building you see today was designed by architect Gustave E. Steinback and is .  Construction began in 1917 and the church was consecrated in 1920.  Designed by Gustave E. Steinback, it is renowned for its Gothic Revival architecture.  The building features a stone facade with arched ridges, a prominent rose window, and a richly decorated interior with stained glass panels.

The church is especially known for its choir, which has performed since 1887 and provides music for the 12:30 PM Sunday Mass, often featuring Gregorian chant, Renaissance works, and classical pieces. The parish operates the School of the Blessed Sacrament, offering education for Pre-K through 8th grade. The church serves the community through worship, pastoral care for the sick, and community outreach. It has served as a notable spiritual hub on the Upper West Side for over a century.
